Ear Surgery

(Otoplasty)

Protruding ears are a source of embarrassment and mental anguish for many people, especially children. Otoplasty is a surgical procedure designed to correct this condition.

How is otoplasty performed?

There are several surgical procedures which are designed to bring the ears closer to the head.  A basic procedure involves folding the cartilage and using sutures to keep the folds together.  After reshaping the cartilage, the incisions are closed with small sutures, and the ears are covered with bandages.  Otoplasty can be performed under general anesthesia in which the patient is put to sleep or under local anesthesia in which the areas are numbed and the patient remains awake but sedated under the supervision of the anesthesiologist.

What can be expected following surgery?

Pain or discomfort connected with otoplasty,usually minimal, is controlled by medication when necessary. Bandages are removed within a few days and may be replaced by a somewhat lighter dressing which is worn for several weeks.  Scars from Otoplasty | Ear Surgery | New Jersey | NJthe incisions are, for the most part, inconspicuous because they are made within the creases of the ears.  Although patients are usually up and around one or two days after surgery, the decision as to when normal activities can be resumed is determined during the first few postoperative visits and is based on the extent of surgery and the patient's healing process.

At what age should otoplasty be performed?

Otoplasty may be performed on anyone over the age of five or six years old when ear growth is almost complete.  For those children with severely protruding ears, it is recommended that the surgery be performed at an early age before they are subjected to psychological stress related to the condition.

Nasal Surgery

(Rhinoplasty)

Nasal reconstruction or rhinoplasty, one of the first cosmetic procedures ever developed, is among those which are performed most frequently today. Thousands of people each year undergo rhinoplasty in order to improve the appearance of their noses and to help them breathe easier.  Typical reasons for this surgery are to improve the angle or reduce the size of the nose or to remove a bump, conditions which may have been the result of natural causes such as genetics, ethnic variations, external trauma to the face or previous rhinoplasty.

What is rhinoplasty? 

Rhinoplasty is a surgical procedure in which deformities of the nose are corrected by removing, rearranging or reshaping bone or cartilage.  Prior to the surgery, careful Nasal Surgery | Rhinoplasty | New Jersey | NJexamination as well as photographs of the nose help to plan the appropriate procedures to obtain the desired cosmetic and functional results.  The surgery can be performed using local anesthesia in which the area is numbed and the patient remains awake or under general anesthesia in which the patient is put to sleep.  For most corrections, an incision is made inside the nose through which the surgeon works to reshape its contour.  Following the surgery, which lasts from an hour and a half to two hours or more, a lightweight splint is applied to maintain the new shape of the nose.  The splint is usually removed within 7-10 days following surgery.  If work is done on the nasal septum, nasal tampons (pads) may be inserted at the time of surgery to protect the septum, the dividing wall between the two chambers of the nose. This packing is removed within a day or two.  For cosmetic rhinoplasty, Dr. Rosenberg may place temporary tampons, but these are removed prior to discharge home.

What can be expected following surgery? 

Discomfort associated with the procedure is usually minimal and may consist of a slight headache, which is easily controlled by medication.  If bruising around the eyes occurs, it will begin to fade within a few days.  Dr. Rosenberg encourages his patients to start using Arnica montana, a homeopathic remedy which is very effective in preventing and treating bruises, the day prior to surgery and to continue for three days following surgery.  All discoloration usually disappears within a week or so.  Swelling around the treated area is to be expected and will slowly diminish.  However, some slight swelling may be present for as long as two or three months, and final results may not be apparent for up to one year.  Patients are usually up and around the day after the procedure, however patients are encouraged to refrain from returning to vigorous exercise for at least two weeks.

How much improvement can be expected from this procedure? 

The amount of improvement is individual and depends upon the extent of the surgery.  In some cases, as when a badly misshapen nose is reconstructed, dramatic results may be obtained.  The removal of a slight bump, on the other hand, may produce very subtle yet satisfying results for the patient.  Prior to surgery, Dr. Rosenberg and the patient will discuss the desired objectives of the surgery and how the nose should look in relation to the patient's other facial features.  He will then plan the procedure with these goals in mind, using digital photography and computerized morphing technology to help the patient understand which changes are possible and to help communicate these potential changes visually in addition to verbally.

Eyelid Surgery

(Blepharoplasty)

Unfortunately, some men and women find their appearance to be tired with drooping Eyelid Surgery | Blepharoplasty | New Jersey | NJeyes, even with a good night's sleep. Blepharoplasty or eyelid surgery can help to restore more youthful, well-rested and energetic look to such individuals.

What is Blepharoplasty?

Blepharoplasty is eyelid surgery which involves removal of fat and excess skin and muscle from the upper and lower eyelids. This type of procedure is generally used to correct drooping or puffy eyelids.  It does not correct crow's feet lines, droopy eyebrow skin or dark pigmentation of lower lids.

How is it performed?

Prior to the surgery the eyelid skin will be marked for incisions.  The incisions are made in the fine creases along the eyelid.  A local anesthetic is generally applied to control local bleeding and bruising.  The patient is placed under intravenous sedation for this procedure, although sometimes it is preferable to use general anesthesia.  A thin crescent of excess skin is removed, muscle and fat deposits may also be removed.  The lower eyelid incisions are placed just below the eyelash line.  Very fine sutures are used for both sites.

What can be expected following surgery? 

Recovery will depend upon whether or not this procedure was done alone or in combination with a browlift or facelift.  After surgery the area around the eye will be mildly swollen for up to two weeks.  Cold compresses (light weight) should be applied and the head should be kept elevated.  Eye rest is recommended. Eye antibiotic drops may be prescribed.  Pain medication and oral antibiotics may be prescribed.  Sutures are generally removed after 4-5 days.  Make-up can be worn after 10 days and contact lenses after about two weeks with doctors approval.

Facelift

(Rhytidectomy)

As people grow older, the natural aging process, genetic influences, exposure to the sun, smoking and other factors cause the skin to wrinkle and sag.  In the younger individual, the face is firm and smooth due to fatty tissue directly beneath the skin.  The tissue which fills out the contours of the face gives it an even, rounded appearance.  As an individual ages the skin begins to sag and to fit more loosely.  Skin folds become more prominent, particularly around the chin, on the jaw line and on the neck.  Rhytidectomy, or facelift, is designed to correct these conditions and give people the youthful appearance they desire.  Often this procedure is done in conjunction with other facial cosmetic procedures such as liposuction, browlift, blepharoplasty (eyelid surgery), chin augmentation and rhinoplasty (surgery of the nose).

How is rhytidectomy performed?

The basic facelift is a procedure in which facial and neck skin and muscles are tightened. The surgery can be performed under general anesthesia in which the patient is put to sleep or local anesthesia in which the area is numbed and the patient remains awake.  The surgeon, working on one side at a time, makes small incisions inside the hairline at the temple, around the front of the ear to behind the earlobe and up to the scalp.  Loose skin is separated from underlying tissue and is pulled up and back, and excess skin is removed. Connective tissue and sagging muscles are tightened, and in some cases, fat deposits are removed from beneath the chin and neck.  Tiny sutures are used to close the incisions.  Scars from the sutures are, for the most part, inconspicuous, because the incisions are hidden within the hairline and the natural creases around the ears.  After surgery, loose bandages are applied to the area. These are removed within a few days.

What can be expected following surgery? 

Rhytidectomy has been improved over the past few decades and is considered a very safe as well as successful procedure with relatively few risks involved. Pain medication is rarely necessary, but is given should the need arise.  A tightness or numbness of the treated area may also be present for a while, but this is self limited.  For several weeks after the surgery, patients are advised to minimize exposure to the sun as much as possible and to wear sunscreen when going out of doors.

What kind of improvement can be expected? 

It has been said that a facelift can make an individual look five, even ten years younger, however the amount of improvement varies and depends upon the initial condition of the patient and the extent of surgery.  In most cases, a single procedure will achieve the desired results while in others, additional procedures may be indicated.  Most patients who have had a facelift have been delighted with their more attractive, younger appearance.

Facial Implants
(Gore S.A.M.)

Gore S.A.M., also known as Gore subcutaneous augmentation material is frequently used in plastic and reconstructive surgery of the face.  It is inserted subcutaneously and augments the contour of the face.  Gore S.A.M. is made of a material consisting of nontoxic polymers that are used in many different kinds of medical products use for implantation throughout the body.  This material is not rejected by the body, so implants made with it can remain in place permanently and is well tolerated by the body.  This implant is made of a porous material; which allows for the body's tissue to grow into it.

How is the procedure performed?

The procedure is done at the office, in our O.R. suite.  Dr. Rosenberg will make a small incision at the implantation site.  Tissue from the area will be gently lifted and small tunnel or pocket is then created for the implant.  The implant is sized and trimmed to fit the patient's particular facial contours.  The implant is then placed into the pocket and the small entry incision sites are closed with rapidly dissolving sutures.  The overall time of the procedure depends on which area of the face is being treated, but the average operative time is under one hour.

What can I expect following the surgery?

Immediately following the surgery there may some discomfort including aching, numbness, and swelling; that may last for about one week.  Dr. Rosenberg will prescribe both antibiotics and a pain medication which should relieve much of the discomfort.  Although the Gore S.A.M. is compatible with your body's natural tissue, it is still an artificial material and may feel different to the touch after surgery.  As with any surgery there are possible risks or side effects. Possible adverse effects include swelling and bruising on or below the skin, infection, extrusion of the implant, induration (hardening of the area operated on), and inadequate or excessive facial contouring.  Proper selection of the appropriate implant shape and size, as well as careful attention to pre and postoperative instructions is the best way to prevent any complications.

Forehead Lift

(Browlift)

In the past cosmetic surgery was reserved for people in their fifties or older. Today with advances in procedures individuals from 35-50 are now seeking facial rejuvenation.  Unfortunately, as we age the upper third of the face, just above the upper eyelids to the hairline begins to descend, leaving a tired, wrinkled appearance to the individual.

What is a browlift?

A browlift or forehead lift is a procedure that reverses the tired, angry look which appears across the forehead over time.  There are two approaches to this procedure, Browlift | Forehead Lift | New Jersey | NJthe traditional method and the more recent endoscopic browlift.  The endoscopic approach provides an indirect approach of repositioning the scalp in an upward direction.

How is it performed?

The endoscopis browlift is performed using a tiny fiberoptic lens inserted through tiny incisions located in the hairline.  The lens is attached to a camera, which connects to a T.V. allowing for visualization.  The surgeon is then able to work on muscles and free up attachments of the eyebrow above the orbital rims.  This allows for the muscles and attachments to be shifted upward , and the forehead is then anchored directly to the skin.  This procedure is beneficial because it minimizes loss of sensation or hair at the incision site.

What can I expect following the procedure?

Immediately following the browlift a dressing will be placed around the entire head.  Many patients may develop a headache due to tension placed on the scalp.  Pain medication and antibiotics will be given.  The patient will not be able to wash hair until after the first post-op follow up.  Ice compresses can be applied to eyes to diminish bruising and swelling.  Removal of clips from scalp can be removed on the fifth or sixth day.  Patients should be able to return to work within ten days to two weeks following surgery.
